Intake forms/paperwork
Many aspects of your life, including your diet, sleep patterns, and stress levels, can impact the appearance and overall well-being of your skin. So, it’s important to honestly and completely answer any questions in these forms about your medical history, pre-existing conditions, medications, supplements, allergies, and lifestyle habits. We generally encourage you to fill out the online intake form, located at www.puamanumedspa.com, under the Patient Info dropdown menu, and click on the New Patient Form.
Consultation
One of our highly qualified providers will meet with you to review some of your answers from our questionnaires/intake forms. Be prepared to answer additional questions about your overall health, skin history, current skin care routine, and goals for your skin and overall wellness.
Evaluation
During your consultation, we will discuss any concerns that you may have about your appearance and health. During the evaluation part of your visit, we take a closer look at the visible aesthetic concerns. Our providers utilize their extensive knowledge and experience to assess these conditions or concerns by sight and touch, as appropriate. Additionally, our medical spa features the latest technologies and specialized instruments to magnify and better visualize areas of concern, including fine lines, dark spots, inflammation/redness, active acne, and large pores.
Recommendations
All information collected from the online form, discussion with you, and as part of a physical exam is then used to make skin care recommendations and, as needed and desired, for professional treatments. These recommendations may include products specifically formulated for your skin type and concerns. We can also discuss potential in-office therapies to try. We welcome any questions that you may have about pre-treatment, the treatment process, aftercare, possible risks, and results from specific therapies. Lifestyle recommendations may be suitable for addressing both cosmetic concerns and wellness-oriented issues, such as poorly managed stress and inadequate sleep quality.

What if I’m sick the day of my appointment?
We understand that life happens. Contact us immediately if you are feeling unwell. Our priority is to keep everyone in our community safe and healthy by minimizing the spread of viruses and sicknesses. This fundamental aspect of our practice aligns closely with our overall vision of upholding the highest standards of safety and quality in our care. We can accommodate you during the next available appointment and when you are feeling better.
Will insurance cover my visit?
At Pua Manu Medspa, we kindly inform our clients that we do not accept health insurance. All treatments are available on a self-pay basis. Thank you for your understanding.
How can I afford treatment?
We offer a variety of payment options to help you achieve your cosmetic and/or wellness goals. We are pleased to provide financing through Cherry. With Cherry, you can finance up to $35,000 in treatment and spread the cost over several weeks and months – up to two years. Depending on your terms, your treatment can be financed at 0% interest. There is also no hard credit check, so no “ding” to your credit should you apply. You can also apply for financing through CareCredit® with no impact on your credit score, too. CareCredit® offers promotional financing options (no interest) for purchases of $200 or more, and when the balance is paid off within the promotional period.
Why do you need to know so much about my health history?
You may seek out our expertise to treat oily, acne-prone skin or sun damage. Even when we strive to help you with conditions that seemingly have nothing to do with other parts of your body, it is still important for us to know about your overall health status. The overall health of your body influences the appearance and health of your skin. So, we need to understand the potential risks associated with treating skin among diabetic patients, for instance. There are unique health risks and considerations related to these and other conditions. We need to know as much as possible about your health to recommend the safest and most effective treatments for you.
Why does it matter what supplements I am taking?
There may be interactions between the skincare products and therapies we recommend and any supplements you may be taking. This notion also applies to any OTC and prescription medications you take for diagnosed conditions or health concerns. We want to avoid potentially dangerous interactions, so this knowledge also supports your overall health and well-being.
How can I prepare for my first visit to your office?
Please allow plenty of time to arrive at our office. That way, you won’t feel rushed when filling out online forms or stressed out before your appointment. Pua Manu MedSpa is designed to be your refuge from day-to-day stressors – and not to be an additional source of stress. It is also helpful to jot down a list of any medications, supplements, allergies, and other important health and lifestyle items, so nothing is missed that our providers may need to know about when conducting their evaluation and making recommendations. Before your appointment, we encourage you to consider your goals and what you hope to achieve through our partnership.
What should I wear?
Leave your restrictive clothes at home. Think: comfortable, relaxed yoga pants, sweatshirts, leggings, and T-shirts. Your comfort is a priority. These are also good options from a practical perspective, because it’s easier for our team to evaluate certain conditions on the body when wearing fabrics and garments that can be easily adjusted. When undergoing any facial evaluation, please leave your heavy makeup at home. Our providers need to see your actual skin, so arrive with a clean, makeup-free face.
